PI = Sum of all scores / Number of surfaces examined. Scores 0–3 per surface. Interpretation: 0=excellent, 0.1–0.9=good, 1.0–1.9=fair, 2.0–3.0=poor.

How is plaque scored?
Score 0 = no plaque. Score 1 = thin film at gingival margin. Score 2 = visible plaque. Score 3 = heavy plaque, filling interdental space.
How many surfaces are examined?
Typically 4 surfaces per tooth × number of teeth examined. A full mouth exam covers up to 128 surfaces.
